Columbia Memorial Hospital now has Oregon Advanced Healthcare Directive booklets. You can ask for a booklet at any admissions desk. The health directive allows you to appoint two representatives to speak on your behalf and make medical decisions for you in the event you are not able to communicate with medical staff.
